Form available at:  http://dyerchamber.com/news/chamber-news/2009-nomination-forms.html  Selection Criteria: Man/Woman of the Year - Motivation\, civic leadership\, business leadership\, community involvement\, community enrichment\, respect by peers\, and dedication to area growth and development.  Nominee shoud be a Chamber member in good standing or employee of member firm and cannot have received the award in the last 5 years.    Distinguished Community Service Award - Affiliation with civic\, professional\, or community organizations\, awards\, and recognition\, and overall efforts to make a positive contribution to individuals and the community.  Individuals must be a resident of Dyer County. Business of the Year - Categorized by number of employees\, must be in operation at least two years and be retail or service business.  Nominee should exhibit excellence in qualities such as financial soundness and profitability\, business growth\, employee relations\, innovations in product or service\, customer satisfaction\, civic and business leadership\, and involvement of owners and employees in the community.  Must be a Chamber member in good standing.
